ENTRY REQUIREMENTS:

1. Applicant must be a Filipino Citizen;
2. At least Grade 10 completer or ALS Graduate;
3. Applicant must be of good Moral Character; and
4. Applicant must be physically fit for the qualification enrolled in.

DOCUMENTARY REQUIREMENTS:

1. Medical Certificate with chest x-ray and drug test from City Health Office;
2. Barangay Clearance;
3. Photocopy of Birth Certificate (PSA or Civil Registry Certification);
4. Photocopy of Grade 10/ALS Certificate of Completion or High School Diploma; and
5. Two (2) pieces Picture (passport size, white background and with collar)
